When playing from an insert point in Pro Tools, the external deck will go through pre-roll, but stops when it reaches the insert point. It may be the case that machine control works fine when Transport=Machine.

Make sure USD is set to receive a valid clock reference. For example, if the clock reference is set to Video, assure that it is receiving a quality black burst signal and that the second video ref port is properly terminated.
