Pro Tools 5.1.1 quits every time I bounce to disk a 24 bit file (any conversion mode)

In one case, an extension conflict was to blame:-Open the Extensions Manager Control Panel. From the selected set pop-up menu, select "OS 9.x base"click on "Duplicate Set" at the bottom of the window. Name the set whatever you like. Enable the following extensions and restart the computer:-Digidesign Control Panel-DigiSystem INIT-Digidesign Direct I/O -Digidesign DSP Manager -Digidesign Stream Manager-Open Music System -USB Floppy Enabler

• When slaving Pro Tools (with USD) to a 2" deck:-If I start the 2" at the top of the song and monitor back both PT and 2", they phase, meaning they're locked. But if I stop the tape, then start again from anywhere besides the top of the song, the 2 are clearly not in sync.
