In one case, this error was happening on a multi-card Pro Tools|HD system (1 HD Core and 3 HD Process cards). As it turns out, the last card in the system, labeled with an "HD Process" sticker, was in fact an HD Core card (serial number verified this). Exchanging the mislabeled HD Core card for an HD Process card resolved the problem.
