I can't hear audio output from the Korg MR-1

I can't hear audio output from the Korg MR-1.

•Is the MR-1 and the connected equipment powered-on? (See p.12 in the product manual)
•Are the amp or headphone connections incorrect?
•Is the volume set to "0"?
