1) Confirm that the MTP-AV has firmware ver. 1.16 or greater (Firmware 1.13 is reported to have problems). This is a chip, not software or flashrom. Contact MOTU if you have earlier firmware.2) You need to disable the MMC (MIDI Machine Control) parser (reader), located at the bottom right corner of MTP A/V control panel 1.2 (select it and hit the delete key).
