"Pro Tools with TDM requires the TDM mixer plug-in in the Plug-Ins Folder. Disk I/O and d24 systems also require a DSP Farm." Error occurs on first launch of Pro Tools 5.1 TDM after updating from an earlier install (5.0x, 4.x, etc...)

This can happen if the previous Plug-Ins folder has an older version of the 16-bit Optimized Mixer. The Cleanup Code of the 5.1 Installer doesn't move the 16-bit Mixer 1.8.x to the Obsoleted Digidesign Software folder, and this error results.The fix is simply to remove the older 16-bit Mixer, and relaunch.
