When working in a Pro Tools session, insertion follows playback turns itself on and off randomly.

This will happen on a Pro Tools TDM system if Commands Focus is enabled in the Edit Window and the user hits 'N' on the Mac Keyboard. This is the shortcut to enable/disable the Insertion Follows Playback preference. To disable Commands Focus in the Edit Window, deselect the 'a...z' button under the Slip and Grid buttons.
