Verify that all three Avid Unity extensions are in the extensions folder. Avid Fibre Driver 3.01, Avid Unity (1.26 for Unity 1.22PT, 2.1 for Unity 2.1) and JNI PCI Compatibility 1.0. This was seen when running Avid Unity extension 1.26 on 2.1 Unity system.
