Lost connection with machine error when reopening a session that previously had a machine online with Machine Control.

This has been seen before with the Keyspan 28x USB to Serial Converter which has two ports. When you close a session with machine control enabled after switching back and forth between time code chasing and machine control, Pro Tools can randomly switch the port designated as the machine control connection.Workaround: open the peripherals, and change back to the correct port.
