With the ADAT I/O in the system clock noise and hiss is generated when attempting to play back a file.

This is a clock related issue. Quit Pro Tools and turn off all peripherals and initiate the startup sequence. Also, check that all slave clock connections are correct.This can also happen when switching out of Pro Tools to another third party application or Sound Designer. We suggest that the ADAT Interface be removed from the system when working in applications other than Pro Tools.
