Q:
"Pro Tools hardware is not turned on", DAE -9045, or noise from all outputs when Pro Tools is slaved to an external signal set to 48k. Setting the external sync signal to 44.1 solves this problem.

This can occur when long BNC cables are used to slave the 2 Audio Interfaces. Switching to shorter BNC sync cables (shorter than 6 Ft.) can solve this.
