Patch the slate output from ProControl or Control|24 into a mono Aux input in Pro Tools. Create a stereo send across every channel of the Pro Tools mixer including the talkback input you just created. Then create stereo aux tracks for each headphone mix desired and set their inputs to correspond to the different stereo sends you've made. Use the sends to route the desired signal to each headphone mix aux track.
