Q:
MachFive 1.2 does not show up in Vstack in OS X
A:
Try removing the Quad and 5.1 version of MachFive from /Library/Audio/Plug-Ins/VST. Vstack is stereo only and the presence of the surrond VST plugins may prevent even the stereo version from loading.
