My LCD19WS monitor from V7 won't attach to its base

My LCD19WS monitor from V7 won't attach to its base.

There's a 4 inch long extension that's included in the package that goes between the base and the swivel on the monitor. Look for that in a plastic bag lodged into the side of the packing foam.
