Q:
It's possible to get region bin file names to mis-match the files on the hard drive.
 
A:
The region bin does not update the filename after changing the audio file's name on the desktop and pointing Pro Tools to it via the "where is" dialog. This is a known issue in Pro Tools 5.1.1
