"OMS can not open the same application multiple times" error dialog appears when launching Pro Tools 4.0.

This happened when there was a "TDM Plug-Ins" folder and a "Plug-Ins" folder in the DAE folder, with the same plug-in files in both folders. Dragging the TDM Plug-ins folder to the desktop solved this problem. It is also possible that some of the 3rd party plug-ins (Waves, or Lexicon) launch an actual application when PT is launched.
